DOOR PRIZE/RAFFLE

All SMDRA members and attendees are invited to donate a door prize to be raffled off at the event. If you wish to donate a door prize

• Contact Michelle Smith at SMDRA – 303-797-3377 or michelle@smdra.com
• Include your name, company name, phone number, what the door prize is and the estimated value ($20 or more)
• All notification of door prizes must be made by the Friday before the event to ensure sponsor recognition.
• Sponsor recognition includes your name and company name announced at the tournament, listed on the sponsor recognition sign at the tournament and other event signage.

$55/Person for Event – Includes Bowling!
$30/Person for Event – Non-Bowlers
Event includes Lunch, Door Prize Drawing Entry, Networking

SCHEDULE:
Check-In / Buffet: 11am
Practice Bowling: 11am to 12pm
Tournament Start Time: 12pm

PRIZES:
Prize Categories Include: Best Costume – Individual & Team; High/Low Male & Female, Game & Series and door prize drawings.

REGISTRATION GUIDELINES:

• Teams/registrations must be submitted by a SMDRA member, this SMDRA Event is not open to the general public. Family, co-workers and friends of SMDRA members are encouraged to attend; but the team must have a SMDRA member. Maximum of 6 bowlers.
• No registrations will be accepted without payment in full.
• ALL LANE ASSIGNMENTS WILL BE RANDOM and assigned after the tournament is full.
  SMDRA will try to accommodate multiple teams and requests, but cannot guarantee placement.
• Any members who sign up after the event is sold out will be placed on a waiting list.
• Teams consist of five bowlers. Registrations of less than five members will be placed with other members to make up a team.
• Cancellation policy – payment returned in full two weeks prior to the tournament. After September 30, 2021, refunds will not be given for cancellations or no shows as a guarantee is given to Arapahoe Bowling Center.
